Investigation reports into two accidents

The Maritime Safety Authority (MSA) today released investigation reports into the following two accidents.

Ikuna Improvements to lighting at Freyberg Wharf are expected as a result of a MSA report into the July 2003 accident where the 97-metre Ikuna collided with the wharf causing over $200,000 worth of damage.

The report also recommends that the Port review the way vessels are handed over from the Pilot to the Master, particularly when the Master is not familiar with berthing in Auckland.

Pacific Mermaid The Master/Owner of a vessel that collided with a competitor during a surf-ski race on Auckland Harbour in March 2004 will not be allowed to be in command again until he regains his Commercial Launchmaster Certificate of Competency.

A Maritime Safety Authority report shows the accident was caused by the Master’s failure to comply with Collision Prevention Rules that state power-driven vessels must keep out of the way of vessels under oars.
