The Transport Accident Investigation Commission’s final report into the circumstances surrounding the death of a deck crew member on the Captain M J Souza is now available on the Commission’s website www.taic.org.nz. The executive summary is attached for your information. The boat is owned and operated by the Talleys Group/Amaltal Fishing Co Ltd.
I should stress that the role of the Commission is to determine the circumstances and causes of the occurrence with a view to avoiding similar occurrences in the future. Its purpose is not to ascribe blame to any person or agency. The Commission carries out its purpose by informing members of the transport sector and the public of the lessons that can be learnt from transport accidents and incidents.
